Enrollment
– Once annual membership has been paid in full, the annual fee is NOT refundable
– Plan members are obligated to pay for all dental care services at time of service
– Membership is active and coverage begins the day the annual fee is paid in full, effective for 1 year
– Auto renewal is eligible
– Member will place credit card on file
