Enrollment

Once annual membership has been paid in full, the annual fee is NOT refundable
Plan members are obligated to pay for all dental care services at time of service
Membership is active and coverage begins the day the annual fee is paid in full, effective for 1 year
Auto renewal is eligible
Member will place credit card on file
